Private Studio & Artist Discretion

I understand that JINDHAMMA is a private, appointment-only tattoo studio operating in Washington State.

Submission of this form does not create a contract, guarantee service, or obligate the artist to accept my request.

JINDHAMMA reserves the sole discretion to accept or decline any request in accordance with Washington law.*

Custom Artwork & Intellectual Property

I acknowledge that all tattoos created by JINDHAMMA are original, custom artworks and are protected under U.S. copyright law.

Reference images are provided for inspiration only and will not be copied or reproduced, in whole or in part.*

Deposit Policy & Reservation of Time

I understand that a non-refundable deposit is required to reserve an appointment time.

I acknowledge that the deposit compensates the artist for reserved time, design preparation, and lost booking opportunities, and is forfeited in accordance with studio policy.*

Pricing Disclosure

I acknowledge that any pricing discussed prior to the appointment is an estimate only.

Final pricing is determined at the time of service based on actual size, placement, complexity, and time required, consistent with Washington consumer disclosure standards.*

Health & Medical Certification

I certify that I am not pregnant or nursing, and that I do not have any medical or skin conditions that would reasonably prevent tattooing, including but not limited to active infections, rashes, sunburn, open wounds, or compromised immune conditions.

I agree to disclose any relevant medical information prior to service.*

Assumption of Risk

I understand that tattooing involves inherent risks, including pain, swelling, infection, allergic reaction, scarring, and variable healing outcomes.

I voluntarily assume all known and unknown risks associated with the tattoo procedure.*

Aftercare Responsibility

I acknowledge that proper aftercare is my responsibility, and that failure to follow aftercare instructions may affect healing and final results.

I agree that the artist is not liable for complications resulting from improper aftercare.*

Cancellations, Rescheduling & No-Show Policy

I understand that late cancellations, rescheduling, or failure to appear may result in forfeiture of my deposit and may impact my eligibility for future bookings.*

Truthfulness & Legal Effect

I certify that all information provided is true and accurate.

I understand that providing false or misleading information may result in refusal of service without refund, as permitted by Washington law.*

Legal Acknowledgment
